It may be necessary to provide medical and/or surgical procedures which are not anticipated for the safety and care of your pet. Failure to be reached on the day of procedure may result in the postponement of the surgery/treatment. Please INITIAL ONE option that will allow us to handle your pets decisions in his/hers best interest (i.e.., tooth extractions, IV fluids, additional pain medication, etc.) if needed.
Preanesthetic blood work is required for your pet to be able to go under anesthesia. There will be a charge of 61.13.

To place an IV catheter. This is recommended for any anesthetic procedure, and may be required for certain surgeries and 8 years and older. ($36.50)
Cerenia injections: We recommend each pet receive Cerenia, which helps alleviate the common anesthesia side effects of nausea and vomitting. This 24 hour lasting injection costs approcimately $37 for a dog/cat that weighs 10lbs, $43 for 50lbs and $53 for 85lbs.
Convenia injections: We recommend this antibiotic injection for patients that have extracted or infected teeth. This antibiotic injection will last 2 weeks. If you are interested in this injection, please ask the receptionist for price as it is weight-based.
Dental X-Rays: We recommend full mouth dental x-rays for all pets having dental procedures done. This procedure allows us to detect disease that is hidden below the gum line. Cost for this is $50.
MICROCHIP: If your pet has not been microchipped, now is the time to do so. We offer a 10% discount off microchips when they are implanted while under anesthesia. This permanent chip is registered with the information you provide to get your pet back if ever lost.
Laser Treatment: Laser therapy is a new modality used to alleviate pain and aid in the healing process. We offer this procedure after surgeries for a cost of $15.
Capstar is given to ALL pets at check in. This is a 24 hour fast acting flea pill that does not affect monthyl flea prevention. Cost is $6.50.
I certify that I am the owner and responsible party for the above named animal, and that having understood the risks involved, including death, have the authority to grant you my consent to receive, prescribe for, treat, induce anesthesia, and/or operate on my pet. All charges, including boarding costs (if necessary), shall be paid upon the release of my pet from AMC.
