Inclement Weather Boarding Waiver: Your pet/pets are important to us and we must do everything in our power to insure their safety and make their stay as comfortable as possible. During times of inclement weather (storms, hurricanes, etc.) it is stressful to not only your pet but the staff as well. Oftentimes the weather does not allow us to take your pets out as often as they are used to and/or not at all. If, in the event the weather is bad enough to keep the staff from coming to work, your pet may go hours without getting properly cared for. That means they could possibly not get walked, fed, watered, or medicated. The lack of proper care could lead to dehydration, illness, or even death. I am aware that Animal Medical Center is recommending that I pick my pet up due to possible inclement weather and I am choosing to leave my pet/pets in boarding knowing the possible risks with inclement weather approaching and will be charged $ 75.00 a day while pet is here during inclement weather times. I am releasing any and all liability from Animal Medical Center if in the event that my pet/pets become ill, harmed, or deceased due to inclement weather.
I assume responsibility for all charges in the care of this animal. I also understand that these charges will be paid at the time of release and that a deposit may be required for surgical treatment of this animal. I understand that if I do not pay for a service as agreed, I agree to pay all the cost of collection. If you choose to have your pets board together, you will be responsible for any incidents that happen between them during their stay.
By signing this form, I acknowledge that I understand Animal Medical Center's requirements, recommendations, and additional charges for my pet's stay.
